CORONAVIRUS DIARIES XIII

Today, I am interviewing a medical expert, researcher, published author of many scientific, peer-reviewed articles and medical textbooks, a well-known professional, and an academic of many years, who currently works for a government agency that supervises medical care in Australia, ensuring that the public are protected and that they enjoy safe, effective and evidence-based treatment and care. I shall refer to this person as Dr X.
 
Jammy: Dr X, thank you for agreeing to this interview on the COVID-19 pandemic. Your expertise is valued and your time is appreciated. 

Dr X: you’re welcome, I am happy to provide as much up-to-date and accurate information as I can in a language that an average lay person can understand. 

Jammy: Please explain to me, what are viruses? 

Dr X: Viruses are interesting aggregates of organic matter, that technically are not able to be classified as “living”. They don’t respire, they don’t respond to stimuli, they cannot reproduce themselves and many of them, if purified, can be crystallised like sugar or salt and stored in a jar on a shelf, unchanging for many years. They are of two major types, depending on whether they possess their genetic material in the form of RNA or in the form of DNA – not both like our cells have. 

Jammy: So are they cells? 

Dr X: No, they are not cellular. They are very simple compared to cells possessing in most cases only a little genetic material in their core and a surrounding, protecting shell of protein, with or without an external envelope of lipid, depending on the virus type. They are exceedingly small. A red blood cell of a human is 7,500 nanometres (a nanometre is a billionth of a metre), a typical bacterium is around 1,000 to 2,000 nanometres, and a large, complex virus is only around 400 nanometres, while a small one is about 25 nanometres. 

Jammy: So if they cannot reproduce themselves, how do viruses multiply? 

Dr X. They are expert fraudsters and master deceivers. When viruses come into the body, their external proteins latch onto cell receptor molecules and thus they enter into living cells. Inside these, they take over the metabolism of the cell and they force the cell to make more and more viruses instead of more cell. As the cell fills up with viruses, it bursts, is destroyed and thousands of new viruses emerge, to infect more body cells, or come out of the body to infect other people. 

Jammy: Hmmm, seems like a pretty pointless existence… 

Dr X. Well, no more pointless than many living organisms, or even some people! 

Jammy: When viruses come into the body, can’t we take antibiotics to destroy the viruses? Just like we do with bacterial infections? 

Dr X. Bacteria, fungi and protozoa can be relatively easily managed with antibiotics and other drugs because they are living organisms with their own metabolism, which in many cases is quite different to human cells. Antibiotics interfere with the metabolism of these microorganisms, killing them or suspending their growth so the body’s immune cells can destroy them. Viruses, as we said, are not alive and do not metabolise. Thus they are not susceptible to antibiotics and most antimicrobial drugs. 

Jammy: But AIDS is caused by a virus and HIV infection can be treated effectively nowadays with drugs, can’t it? 

Dr X: Yes, HIV infection can be effectively managed nowadays with a cocktail of specific drugs because HIV is a rather special virus. It is a virus which can only make the cell manufacture more virus by getting the cell it infects to first make a special enzyme that doesn’t exist in human cells (the enzyme is called reverse transcriptase). Many of the anti-HIV drugs interfere with this special enzyme’s activity in cells, hence preventing viral replication in cells. 

Jammy: So, theoretically, it’s possible to have a drug that interferes with COVID-19 replication in cells? That would get rid of virus from the body, and hence infection? 

Dr X: This is much more difficult. COVID-19 is a more or less quite ordinary virus, which comes into cells and takes over cell metabolism easily, utilising all of the cell’s own enzymes and nutrients to make more virus. If we interfere with these cellular metabolic pathways with a drug, we would be interfering with the metabolic process of all cells in our body, which could effectively kill us. A rather drastic way of overcoming a viral infection. 

Jammy: What about hydroxychloroquine? Doesn’t that help with overcoming COVID-19 infection? 

Dr X: For a while, some initial studies with it showed promise. Unfortunately, examination of these initial trials with this drug, indicated that they were conducted in a rather haphazard manner and the results of the studies were not interpreted in a scientific manner. More trials were conducted, and at this time, there are very limited data to support the use of hydroxychloroquine for the treatment or prevention of COVID-19. Clinical evidence is emerging, but results are inconclusive. Besides, prolonged use of hydroxychloroquinone (especially in compromised patients or together with other drugs) has numerous side effects, some of which are life-threatening. Hydroxychloroquine is definitely not the wondrous cure for COVID-19 as some very vocal people are vehemently suggesting – I wonder if these people have shares in drug companies manufacturing hydroxychloroquine? 

Jammy: What about a vaccine against COVID-19? 

Dr X: Vaccines are the standard, safe, cheap and effective way to prevent a whole variety of different viral diseases, for example: Polio, measles, hepatitis B, rubella. Will a vaccine be developed as easily for COVID-19 as for the diseases I just mentioned? The answer is maybe yes, maybe not. The “maybe yes” comes from the observation that in animal studies, coronaviruses stimulate strong immune responses, which seem capable of knocking out the virus. Recovery from COVID-19 may be in large part due to effective immune response. The “maybe not” comes from evidence just as strong, at least with earlier SARS and MERS viruses, that natural immunity to these viruses is short-lived. In fact, some animals can be reinfected with the very same strain that caused infection in the first place. 

Jammy: Some people suggest that we should not bother with restrictions and precautions and just rely on herd immunity to get us over the pandemic. 

Dr X: Herd immunity occurs when a large portion of a community (the herd) becomes immune to a disease, making the spread of disease from person to person unlikely. As a result, the whole community becomes protected not just those who are immune. Often, a percentage of the population must be capable of getting a disease in order for it to spread. This is called a threshold proportion. If the proportion of the population that is immune to the disease is greater than this threshold, the spread of the disease will decline. This is known as the herd immunity threshold. What percentage of a community needs to be immune in order to achieve herd immunity? It varies from disease to disease. The more contagious a disease is, the greater the proportion of the population that needs to be immune to the disease to stop its spread. For example, measles is a highly contagious illness. It is estimated that 94% of the population must be immune to interrupt the chain of transmission.

There are some major problems with relying on community infection to create herd immunity to the virus that causes COVID-19. First, it isn’t yet clear if infection with the COVID-19 virus makes a person immune to future infection (as we said, that is one of the problems with making a protective vaccine against this virus).

Even if infection with the COVID-19 virus creates long-lasting immunity, a large number of people would have to become infected to reach the herd immunity threshold. Experts estimate that in the USA, 70% of the population (i.e., more than 200 million people!) would have to recover from COVID-19 to halt the epidemic. If many people become sick with COVID-19 at once, the health care system could quickly become overwhelmed. This amount of infection could also lead to serious complications and millions of deaths, especially among older people and those who have chronic conditions. 

Jammy: Oh dear! We are in a bind… So what can we do? 

Dr X: We must slow the spread of the COVID-19 virus and protect individuals at increased risk of severe illness, including older adults and people of any age with underlying health conditions. To reduce the risk of infection we must all:

  • Avoid large events and mass gatherings.
  • Avoid close contact (within about 6 feet, or 2 meters) with anyone who is sick or has symptoms.
  • Stay home as much as possible and keep distance between yourself and others (within about 6 feet, or 2 meters) if COVID-19 is spreading in your community, especially if you have a higher risk of serious illness. Keep in mind some people may have the COVID-19 virus and spread it to others, even if they don’t have symptoms or don’t know they have COVID-19.
  • Wash your hands often with soap and water for at least 20 seconds, or use an alcohol-based hand sanitiser that contains at least 60% alcohol.
  • Wear a cloth face covering or face mask in public spaces, such as in shops, where it’s difficult to avoid close contact with others, especially if you’re in an area with ongoing community spread. Dispose of the face mask safely in a rubbish bin.
  • Cover your mouth and nose with your elbow or a tissue when you cough or sneeze. Throw away the used tissue in a rubbish bin.
  • Avoid touching your eyes, nose and mouth.
  • Avoid sharing dishes, glasses, bedding and other household items if you’re sick.
  • Clean and disinfect high-touch surfaces, such as doorknobs, light switches, electronics and counters, daily.
  • Stay home from work, school and public areas if you’re sick, unless you’re going to get medical care. Avoid public transportation, taxis and ride-sharing if you’re sick.
  • Get tested for COVID-19 if you have symptoms and self-isolate at home until you get the results.

FOOD FRIDAY - CHOCOLATE MOUSSE TIRAMISÙ

I have a sweet tooth – no, perhaps I have several. Maybe even a mouthful of them! I do like my desserts, candies, chocolates, sweetmeats of all kinds. Even after a Lucullan meal of several courses, I don’t seem to feel replete unless my mouth tastes something sweet. In any case it seems I am not the only one because every meal worth its salt finishes off with a dessert course. Some nutritionists claim that people have been trained since childhood to expect a sugary dessert after a full meal. In many families, it’s quite the done thing, and perhaps the way to bribe children into finishing their greens!

But maybe the chemistry of the human brain is to blame for the after-dinner sweet tooth. There is evidence to suggest that eating sugar (or other simple carbohydrates) can enhance the absorption of the amino acid tryptophan found in some foods. The tryptophan then enables an increase in the levels of serotonin, a neurotransmitter associated with feelings of well-being. Hence, bring on the dessert!

For some people, a heavy meal can result in a condition called postprandial (or reactive) hypoglycaemia, a state of low blood sugar that is marked by hunger, weakness, sweating, shakiness, sleepiness, lightheadedness, anxiety or confusion. Consuming sweet foods is one way to counteract the symptoms of reactive hypoglycaemia. The causes of this condition are varied and can be either inherited or acquired.

Well, whatever the cause of your sweet tooth, here is a recipe that hits the spot: 

Chocolate Mousse Tiramisù
Ingredients
4 cups of your standard, favourite recipe chocolate mousse
500 mL of thickened whipping dairy cream
1 cup icing sugar (dissolved in the cream)
9 Savoiardi (lady finger) biscuits
20 mL amaretto liqueur
20 mL maraschino liqueur
250 g mascarpone cheese
Grated dark chocolate for serving

Method
Prepare six glass dessert serving cups by chilling in the refrigerator. Dissolve the amaretto liqueur in about 200 mL of the cream. Break up the biscuits into 3-4 pieces. Dip into liqueur/cream mixture to soften slightly and place one-and-a-half biscuits into each of the prepared serving cups. Any liqueur/cream mixture left over can be poured over the biscuits in the cups.
Spoon the chocolate mousse evenly (3/4 cup) into each of the cups and smooth the surface. Refrigerate. Meanwhile, prepare the mascarpone cream: Soften the cheese in a bowl by stirring with a fork and smooth it up as much as you can. Dissolve the maraschino liqueur to the remaining cream and slowly add the cream into the cheese, little by little while stirring, to incorporate it. Continue to stir until you have a smooth, soft mixture.
Spoon (or pipe) the mascarpone cream over the chocolate mousse. Refrigerate until ready to serve and garnish by grating dark chocolate on top.

MYTHIC MONDAY - EGYPT 33, RESHEPH

Resheph (also Rešef, Reshef; Canaanite ršp רשף; Eblaite Rašap, Egyptian ršpw) was a deity associated with plague (or a personification of plague) in the ancient Canaanite religion. The originally Eblaite and Canaanite deity was adopted into ancient Egyptian religion in the late Bronze Age during the Eighteenth Dynasty of Egypt (late 15th century BC) as a god of horses and chariots. In Biblical Hebrew, רֶשֶׁף‎ resheph is a noun interpreted as “flame, lightning” but also “burning fever, plague, pestilence”.

Ršp was an important Ugaritic deity. He had the byname of tġr špš “door-warden of the Sun”. Sacrifices to Ršp (ršp gn) were performed in gardens. Ugaritic Ršp was equated with Mesopotamian Nergal. One of Ršp’s epithets (ḥṣ) is interpreted as “arrow” and identifies Ršp as a plague god who strikes his victims with arrows as Homeric Apollo (Iliad I.42–55), and is therefore seen to be the Ugaritic equivalent of Apollo.

Resheph was adopted as an official deity in Egypt under Amenhotep II during the Eighteenth Dynasty of Egypt as god of horses and chariots. Originally adopted into the royal cult, Resheph became a popular deity in the Twentieth Dynasty while disappearing from royal inscriptions. In this later period, Resheph often appears with Qetesh and Min. In this time, however, most his stelae are found in Deir el-Medina, a settlement of Syrian (Levantine) craftsmen. The theonym is usually written as hieroglyphic ršpw, where the final -w is added in analogy to other Egyptian divine names.

Resheph was invoked for his power to cure ordinary people’s disease. He was thought to be able to repel the ‘akha’ demon, which causes abdominal pains. He also became the approachable deity who could grant success to those praying to him. Resheph was closely associated with the native Egyptian war god, Monthu and with many other deities. He was also known as “Lord of Sky”, “Lord of Eternity”, “Lord of Heaven” or “Governor of all the Gods” and an area of the Nile valley was renamed the “Valley of Reshep”.

ALL ABOUT LEMON VERBENA

Aloysia citrodora is a species of flowering plant in the verbena family Verbenaceae, native to western South America. Common names include lemon verbena and lemon beebrush. It was brought to Europe by the Spanish and the Portuguese in the 17th century and cultivated for its oil.

The first European botanist who publicly noticed this plant was the French Philibert Commerson, who collected in Buenos Aires on his botanical circumnavigation with Bougainville, about 1767. The plant had already been quietly imported directly into the Real Jardín Botánico de Madrid, where in 1797 professors Casimiro Gómez Ortega and Antonio Palau y Verdera named it, though they did not yet effectively publish it, Aloysia citrodora in Latin and “Hierba de la Princesa” in Spanish, to compliment Maria Louisa of Parma, Princess of Asturias the wife of the Garden’s patron Infante Carlos de Borbon, Prince of Asturias and son of king Carlos III. The name was later effectively published in the first volume of Palau’s Parte Práctica de Botánica in 1784.

Unofficial importations from Spanish America seldom fared well: When French botanist Joseph Dombey landed his collections at Cadiz in 1785 they were impounded and left to rot in warehouses, while he was refused permission even to have seeds planted. Among the bare handful of plants Dombey had assembled during eight years at Lima, lemon verbena survived. Meanwhile, Gómez Ortega sent seeds and specimens of the plant to Charles Louis L’Héritier de Brutelle in Paris; L’Héritier published it as Verbena triphylla in the second fascicle his Stirpes Novae, published in December 1785 or January 1786.

From Paris, John Sibthorpe, professor of Botany at Oxford, obtained the specimen that he introduced to British horticulture: By 1797 lemon verbena was common in greenhouses around London, and its popularity as essential in a fragrant bouquet increased through the following century. The plant has gained the Royal Horticultural Society’s Award of Garden Merit.

 Lemon verbena is a perennial shrub or subshrub growing to 2–3 m high. The 8-cm-long, glossy, pointed leaves are slightly rough to the touch and emit a powerful scent reminiscent of lemon when bruised (hence the Latin specific epithet citrodora—lemon-scented). Sprays of tiny purple or white flowers appear in late spring or early summer. It is sensitive to cold, losing leaves at temperatures below 0°C, although the wood is hardy to −10°C.Due to its many culinary uses, it is widely listed and marketed as a plant for the herb garden.

Lemon verbena leaves are used to add a lemon flavour to fish and poultry dishes, vegetable marinades, salad dressings, jams, puddings, Greek yogurt and beverages. It also is used to make herbal teas, or added to standard tea in place of actual lemon (as is common with Moroccan tea). It can also be used to make a sorbet. In addition, it has anti-Candida albicans activity. In the European Union, Verbena essential oils (Lippia citriodora Kunth.) and derivatives other than absolute are prohibited when used as a fragrance ingredient.

The major isolates in lemon verbena oil are citral (30–35%), nerol and geraniol. Extracts of lemon verbena also contain verbascoside. Aloysia citriodora extract shows antioxidant properties that could play an important role in modulating GSH-reductase activity in lymphocytes and erythrocytes and protecting plasma from exercise oxidative damage. Lemon verbena extract containing 25% verbascoside showed strong antioxidant capacity, especially in a lipophilic environment, which was higher than expected as concluded from the antioxidant capacity of pure verbascoside, probably due to synergistic effects.

In the language of flowers, a sprig of non-flowering lemon verbena carries the message: “We are united”. A flowering sprig means: “You have bewitched me”.

ALL ABOUT WINTER SAVOURY

Winter savoury (Satureja montana) is a perennial herb in the family Lamiaceae, native to warm temperate regions of southern Europe and the Mediterranean. It is a perennial plant growing to 40 cm tall. The leaves are opposite, oval-lanceolate, 1–2 cm long and 5 mm broad. The herb has spike-like clusters of tubular 2-lipped, white flowers in summer. Superficially, this herb resembles a blooming rosemary bush with very pale flowers.

Winter savoury  is easy to grow, and it makes an attractive border plant for any culinary herb garden. It requires six hours of sun a day in soil that drains well. S. montana ‘Nana’ is a dwarf cultivar, which can be grown in well-drained pots. In temperate climates it goes dormant in winter, putting out leaves on the bare stems again in the spring. It is important to not cut the plant back, as all those stems which appear dead will leaf out again. It is hardy and has a low bunching habit. It is used as a companion plant for beans, keeping bean weevils away, and also roses, reducing mildew and aphids.

 Winter savoury has been used in the garden, kitchen and apothecary’s shop for hundreds of years. Both this herb and summer savoury (Satureja hortensis) have been grown and used, virtually side by side. Both have strong, spicy flavours. It goes particularly well with any type of mushroom, or in white sauces, and is very good in potato salads. Small amounts spice a regular salad well. We add the herb to beans and meats, especially lighter meats such as chicken or turkey, and can be used in stuffings. It has a rich herbaceous aroma when crushed, however, it should be noted that the intense flavour is lost when the herb is cooked.

Winter savoury has been purported to have antiseptic, aromatic, carminative, and digestive properties. It has also been used as an expectorant and in the treatment of stings. The plant has a stronger action than the closely related summer savoury. Taken internally, it is said to be a remedy for colic and a cure for flatulence, whilst it is also used to treat gastroenteritis, cystitis, nausea, diarrhoea, bronchial congestion, sore throat and menstrual disorders. It should not be prescribed for pregnant women.

A sprig of the plant, rubbed onto bee or wasp stings, is said to bring instant relief. Therapeutic-grade oil has been shown to inhibit the growth of Candida albicans. The plant is harvested in the summer when in flower and can be used fresh or dried. The essential oil forms an ingredient in lotions for the scalp in cases of incipient baldness. An ointment made from the plant is used externally to relieve arthritic joints. In traditional herbal medicine, summer savoury was believed to be an aphrodisiac, while winter savoury was believed to inhibit sexual desire.

The herb in the language of flowers has meanings that relate to curbing of carnal desire. A sprig of the non-flowering herb means: “My intentions are honourable”. A sprig of the flowering herb says: “My interest in you is purely platonic.”

ALL ABOUT SOUTHERNWOOD

Artemisia abrotanum (southernwood, lad’s love, southern wormwood) is a species of flowering plants in the Asteraceae family. It is native to Eurasia and Africa but naturalised in scattered locations in North America. Other common names include: Old man, boy’s love, oldman wormwood, lover’s plant, appleringie, garderobe, Our Lord’s wood, maid’s ruin, garden sagebrush, European sage, sitherwood and lemon plant.

Southernwood has a strong camphor-like odour and was historically used as an air freshener or strewing herb. It forms a small bushy shrub, which is widely cultivated by gardeners. The grey-green leaves are small, narrow and feathery. The small flowers are yellow. It can easily be propagated by cuttings, or by division of the roots. This plant has gained the Royal Horticultural Society’s Award of Garden Merit.

A yellow dye can be extracted from the branches of the plant, for use in dying wool. Its dried leaves are used to keep moths away from wardrobes. The volatile oil in the leaves is responsible for the strong, sharp, scent that repels moths and other insects. It was customary to lay sprays of the herb amongst clothes, or hang them in closets, and this is the origin of southernwood’s French name, “garderobe” (‘clothes-preserver’). Judges carried posies of southernwood and rue to protect themselves from prisoners’ contagious diseases, and some church-goers relied on the herb’s sharp scent to keep them awake during long sermons.

The pungent, scented leaves and flowers are used in herbal teas. Young shoots were used to flavour pastries and puddings. In Italy, it is used as a culinary herb, especially with fatty meats. It has a strong, overpowering flavour and should be used sparingly in cooking.

In herbal medicine, Southernwood was used as an emmenagogue. It was said to be a good stimulant tonic, possessing some nervine principle. It was given in infusion of 1 ounce of the herb to 1 pint of boiling water, prepared in a covered vessel, the escape of steam impairing its value. Apparently, this type of infusion or tea is agreeable, but a decoction is distasteful, having lost much of the aroma. Considerable success was also attributed to it as an anthelmintic, being chiefly used against the worms of children, teaspoonful doses of the powdered herb being given in treacle morning and evening.

Southernwood should not be used by pregnant women. Some people are allergic to Southernwood and one should exercise caution in its use. People with hay fever may find its pollen bothersome and some experience contact dermatitis from the plant.

In the language of flowers, a sprig of southernwood foliage means “constancy”. A flowering stem means “A jest; good humour”.

ALL ABOUT TANSY

Tansy (Tanacetum vulgare) is a perennial, herbaceous flowering plant of the aster family, native to temperate Europe and Asia. It has been introduced to other parts of the world including North America, and in some areas has become invasive. It is also known as common tansy, bitter buttons, cow bitter, or golden buttons. Tansy is absent from Siberia and some of the Mediterranean islands. The ancient Greeks may have been the first to cultivate it as a medicinal herb. In the sixteenth century it was considered to be “necessary for a garden” in Britain.

The plant is a flowering herbaceous species with finely divided compound leaves and yellow, button-like flowers. It has a stout, somewhat reddish, erect stem, usually smooth, 50–150 cm tall, and branching near the top. The leaves are alternate, 10–15 cm long and are pinnately lobed, divided almost to the centre into about seven pairs of segments, or lobes, which are again divided into smaller lobes having saw-toothed edges, giving the leaf a somewhat fernlike appearance.

The roundish, flat-topped, button-like, yellow flower heads are produced in terminal clusters from mid-to-late summer. The scent is similar to that of camphor with hints of rosemary. The leaves and flowers are toxic if consumed in large quantities; the volatile oil contains toxic compounds including thujone, which can cause convulsions and liver and brain damage. If you intend to use tansy as a culinary herb do not use it to excess and do not use it at all if you are allergic to it. Some insects, notably the tansy beetle Chrysolina graminis, have resistance to the toxins and subsist almost exclusively on the plant.

Tansy has a long history of use. It was first recorded as being cultivated by the ancient Greeks for medicinal purposes. In the 8th century AD it was grown in the herb gardens of Charlemagne and by Benedictine monks of the Swiss monastery of Saint Gall. Tansy was used to treat intestinal worms, rheumatism, digestive problems, fevers, sores, and to bring out measles. During the Middle Ages and later, high doses were used to induce abortions. Contrary to this, tansy was also said to help women conceive and to prevent miscarriages. In the 15th century, Christians began serving tansy with Lenten meals to commemorate the bitter herbs eaten by the Israelites. Tansy was thought to have the added Lenten benefits of controlling flatulence brought on by days of eating fish and pulses and of preventing the intestinal worms believed to be caused by eating fish during Lent.

Tansy was used as a face wash and was reported to lighten and purify the skin. In the 19th century, Irish folklore suggested that bathing in a solution of tansy and salts would cure joint pain. Although most of its medicinal uses have been discredited, tansy is still a component of some medicines and is listed by the United States Pharmacopeia as a treatment for fevers, feverish colds, and jaundice.

Tansy has also been cultivated and used for its insect repellent and in the worm warding type of embalming. It was packed into coffins, wrapped in funeral winding sheets, and tansy wreaths were sometimes placed on the dead. During the American colonial period, meat was frequently rubbed with or packed in tansy leaves to repel insects and delay spoilage. Tansy was frequently worn at that time in shoes to prevent malaria and other fevers; it has been shown, however, that some mosquito species including Culex pipiens take nectar from tansy flowers.

Tansy can be used as in companion planting and for biological pest control. It is planted alongside potatoes to repel the Colorado potato beetle, with one study finding tansy reduced the beetle population by 60 to 100%. In England tansy is placed on window sills to repel flies; sprigs are placed in bed linen to drive away pests, and it has been used as an ant repellent. In the 1940s, distilled tansy oil mixed with fleabane, pennyroyal and diluted alcohol was a well-known mosquito repellent. Some research studies support these insect-repellent uses.

Tansy was formerly used as a flavouring for puddings and omelettes, but this culinary use is now almost unknown. The herbalist John Gerard (c. 1545–1612) noted that tansy was well known as “pleasant in taste”, and he recommends tansy sweetmeats as “an especial thing against the gout, if every day for a certain space a reasonable quantitie thereof be eaten fasting.” In Yorkshire, tansy and caraway seeds were traditionally used in biscuits served at funerals. During the Restoration, a “tansy” was a sweet omelette flavoured with tansy juice. In the BBC documentary “The Supersizers go ...Restoration”, Allegra McEvedy described the flavour as “fruity, with a sharpness to it and then there’s a sort of explosion of cool heat a bit like peppermint.” However, the programme’s presenter Sue Perkins experienced tansy toxicity. According to liquor historian A. J. Baime, in the 19th century Tennessee whiskey magnate Jack Daniel enjoyed drinking his own whiskey with sugar and crushed tansy leaf.

Many tansy species contain a volatile oil, which can cause contact dermatitis in sensitive individuals. If taken internally, toxic metabolites are produced as the oil is broken down in the liver and digestive tract. It is highly toxic to internal parasites, and for centuries tansy tea has been prescribed by herbalists to expel worms. Tansy is an effective insecticide and is highly toxic to arthropods.

In the language of flowers, tansy leaves mean "the truth is bitter", while flowering stems indicate "hate, bitterness and a declaration of war".

ALL ABOUT RUE

“There’s fennel for you, and columbines: there’s rue for you; and here’s some for me: we may call it herb-grace o’ Sundays: O you must wear your rue with a difference...” – Ophelia in William Shakespeare’s “Hamlet”.

This definition above is one meaning of the word rue, the other relates to the herb that the quote from “Hamlet” is referring to. Shakespeare is of course in punning mode in that quote and the herb rue lends itself well to Ophelia’s sentiments.

Ruta graveolens, commonly known as rue, common rue or herb-of-grace, is a species of Ruta in the family Rutaceae grown as an ornamental plant and as a herb. It is native to the Balkan Peninsula. It is now grown throughout the world in gardens, especially for its striking bluish leaves, and sometimes for its tolerance of hot and dry soil conditions. It is also cultivated as a medicinal herb, as a condiment, and to a lesser extent as an insect repellent.

The name Ruta is from the Greek reuo (“to set free”), because this herb is so efficacious in various diseases. It was much used by the Ancients; Hippocrates specially commended it, and it constituted a chief ingredient of the famous antidote to poison used by Mithridates. The Greeks regarded it as a remedy to magic, because it served to assuage the nervous indigestion they suffered when eating before strangers, which they attributed to witchcraft. In the Middle Ages and later, it was considered (in many parts of Europe) a powerful defence against witches, and was used in many spells. It was also thought to bestow second sight.

The stem of rue is woody in the lower part, and tender in the young shoots. The leaves are alternate, bluish-green, bi- or tripinnate, and emit a powerful, disagreeable (for most people) odour and have an exceedingly bitter, acrid and nauseous taste. I rather like the smell of rue, while my mother dislikes it intensely, and my father is quite indifferent to it.

The greenish-yellow flowers are in terminal panicles, blossoming from June to September in the Northern Hemisphere. The first flower that opens has usually ten stamens, the others eight only. In England Rue is one of the oldest garden plants, cultivated for its use medicinally, having, together with other herbs, been introduced by the Romans, but it is not found in a wild state except rarely on the hills of Lancashire and Yorkshire. This wild form is even more intense in smell than the garden rue.

The plant grows almost anywhere, but thrives best in a partially sheltered and dry situation. Propagation may be effected by seeds, sown outside, broadcast, in spring, raked in and the beds kept free from weeds, the seedlings, when about 4 cm high, being transplanted into fresh beds, allowing about 40 cm each way, as the plants become busy. Alternatively, it is easy to strike cuttings, taken in spring and inserted for a time, until well rooted, in a shady border. It is said the best cuttings to use are those stolen from a thriving plant in some stranger’s garden. However, every slip or cutting of the young wood will readily grow, and this is the most expeditious way of raising a stock. Rue will live much longer and is less liable to be injured by frost in winter when grown in a poor, dry, infertile soil rather than in good ground.

Rue does have a culinary use if used sparingly, but it is bitter and gastric discomfort may be experienced by some individuals. Although used more extensively in former times, it is not an herb that is typically found in modern cuisine, and is today largely unknown to the general public and most chefs, and unavailable in grocery stores. Rue extracts are mutagenic and hepatotoxic! Large doses can cause violent gastric pain, vomiting, systemic complications, and death. Exposure to common rue, or herbal preparations derived from it, can cause severe phytophotodermatitis, which results in burn-like blisters on the skin! One can see why most people eschew it!

Nevertheless, rue is a component of berbere, the characteristic Ethiopian spice mixture, and as such is encountered in Ethiopian cuisine. It was used extensively in ancient Near Eastern and Roman cuisine (according to Ibn Sayyar al-Warraq and Apicius). Rue leaves and berries are an important part of the cuisine of Ethiopia. Rue is used as a traditional flavouring in Greece and other Mediterranean countries.

In Istria (a region in Croatia), and in Northern Italy, it is used to give a special flavour to grappa/raki and most of the time a little branch of the plant can be found in the bottle. This is called grappa alla ruta. Seeds can be used for porridge. The bitter leaf can be added to eggs, cheese, fish, or mixed with damson plums and wine to produce a meat sauce. In Italy in Friuli Venezia-Giulia, the young branches of the plant are dipped in a batter, deep-fried in oil, and consumed with salt or sugar. They are also used on their own to aromatise a specific type of omelette. Rue was also used in Old World beers as a flavouring ingredient.

In the language of flowers, the rue plant stands for sorrow or repentance. Rue flowers mean grace, clear vision. At one time the holy water was sprinkled from brushes made of Rue at the ceremony usually preceding the Sunday celebration of High Mass, for which reason it is supposed it was named the Herb of Repentance and the Herb of Grace.

Rue has been used in herbal medicine from time immemorial. Pliny reported rue to be of such effect for the preservation of sight that the painters of his time used to devour a great quantity of it, and the herb is still eaten by the Italians in their salads. It was supposed to make the sight both sharp and clear, especially when the vision had become dim through over-exertion of the eyes. It was with “Euphrasy and Rue” that Adam's sight was purged by Milton’s Angel in his “Paradise Lost”.

Gerard tells us: “The garden Rue, which is better than the wild Rue for physic’s use, grows most profitably, as Dioscorides said, under a fig tree.” But this is, probably, only a reference, originally, to the fact that the plant prefers a sheltered position. Country-people boil its leaves with treacle, thus making a conserve of them. These leaves are curative of croup in poultry. It has also been employed in the diseases of cattle.

Piperno, a Neapolitan physician, in 1625, commended Rue as a specific against epilepsy and vertigo, and for the former malady, at one time, some of this herb used to be suspended round the neck of the sufferer.

Rue is strongly stimulating and antispasmodic and is often employed, in form of a warm infusion, as an emmenagogue. In excessive doses, it is an acro-narcotic poison, and on account of its emetic tendencies should not be administered immediately after eating. It forms a useful medicine in hysterical affections, in coughs, croupy affections, colic and flatulence, being a mild stomachic. The oil may be given on sugar, or in hot water.

Externally, Rue is an active irritant, being employed as a rubefacient. If bruised and applied, the leaves will ease the severe pain of sciatica. The expressed juice, in small quantities, was a noted remedy for nervous nightmare, and the fresh leaves applied to the temples are said to relieve headache. Compresses saturated with a strong decoction of the plant, when applied to the chest, have been used beneficially for chronic bronchitis. If a leaf or two be chewed, a refreshing aromatic flavour will pervade the mouth and any nervous headache, giddiness, hysterical spasm, or palpitation will be quickly relieved.

ALL ABOUT THYME

Thyme is an evergreen herb with culinary, medicinal, and ornamental uses. The most common variety is Thymus vulgaris. Thyme is of the genus Thymus of the mint family (Lamiaceae), and a relative of the oregano genus Origanum. It is a popular culinary herb and has also many other uses, including the extraction of an essential oil and various useful organic chemicals.

The Ancient Egyptians used thyme for embalming, the high thymol content having a pleasant aromatic odour and strong antiseptic properties. The ancient Greeks used thyme in their baths and burnt it as incense in their temples, believing it was a source of courage. The spread of thyme throughout Europe was thought to be due to the Romans, as they used it to purify their rooms and to give an aromatic flavour to cheese and liqueurs. In the European Middle Ages, the herb was placed beneath pillows to aid sleep and ward off nightmares. In this period, women also often gave knights and warriors gifts that included thyme leaves, as it was believed to bring courage to the bearer. Thyme was also used as incense and placed on coffins during funerals, as it was supposed to assure passage into the next life.

Thyme is best cultivated in a hot, sunny location with well-drained soil. It is generally planted in the spring, and thereafter grows as a perennial. It can be propagated by seed, cuttings, or dividing rooted sections of the plant. It tolerates drought well. The plants can take deep freezes and are found growing wild on mountain highlands. Along the Italian Riviera, it is found from sea level up to 800 m. In the mountains of Greece thyme is widespread and the honey that bees make by harvesting nectar from its flowers has an exceedingly pleasant aroma.

Thyme for culinary use is sold both fresh and dried. The fresh form is more flavourful, but also less convenient; storage life is rarely more than a week. Although the fresh form only lasts a week or two under refrigeration, it can last many months if carefully frozen. Thyme retains its flavour on drying better than many other herbs, so it commonly used dried. It is perfectly acceptable to substitute the dried leaves of the herb for whole fresh thyme. However, one should be careful with quantities, as dried thyme can pack quite a punch!

In some Middle Eastern countries, the condiment za’atar (Arabic for thyme) contains thyme as a vital ingredient, together with other herbs, salt and sesame seeds. Dried thyme is widely used in Armenia in tisanes (called urc). In French cuisine, thyme is a common component of the bouquet garni, and of herbes de Provence. Thyme is one of the herbs used in flavouring the liqueur Benedictine.

Oil of thyme, the essential oil of common thyme (Thymus vulgaris), contains 20–54% thymol. Thyme essential oil also contains a range of additional compounds, such as p-cymene, myrcene, borneol, and linalool. Thymol, is a clear crystalline solid and is an antiseptic. It is an active ingredient in various commercially produced mouthwashes such as Listerine. Before the advent of modern antibiotics, oil of thyme was used to medicate bandages. It has also been shown to be effective against various fungi that commonly infect toenails. Thymol can also be found as the active ingredient in some all-natural, alcohol-free hand sanitisers. A tisane made by infusing the herb in water can be used for coughs and bronchitis.

In the language of flowers, a sprig of thyme without flowers, signifies strength and courage. A sprig of flowering thyme means “rest well and sleep soundly”.

POETS UNITED - HEALING

The theme for this week’s Poets United poetical challenge is “Healing”. To heal is the desired outcome of much treatment, but often nowadays modern medicine is cure-focussed and the patient may suffer much during the attempts at the restoration of health. I remember when at University and later in hospitals, we were told that “to care is the primary objective of the healer, to cure a secondary one.” This I think is often forgotten in modern medicine, which seeks magic bullets, efficacious remedies, wonderful cure-alls and powerful panaceas that cure all ills. Such fantastic treatments may have dire side-effects and often the cure they bring about may exact a terrible price on the patient’s health – one disease cured, but its place taken by another, caused by the medication…

Sterile Moonlight

MOVIE MONDAY - CALL THE MIDWIFE

We have just finished watching the first two seasons (2012-13) of the excellent BBC TV series “Call the Midwife”. The screenplay by Heidi Thomas is based on Jennifer Worth’s memoirs: “Call the Midwife”, later called “Call the Midwife: A True Story of the East End in the 1950s”, is the first in a trilogy of books describing Worth’s work as a district nurse and midwife in the East End of London during the 1950s. Worth wrote the book after retiring from a subsequent career as a musician, and it was originally published in 2002. Reissued in 2007, it became a bestseller, as did the sequel “Shadows of the Workhouse” (2005, reissued 2008) and the final volume “Farewell to the East End” (2009). By the time of Jennifer Worth’s death in June 2011, her books had already sold almost a million copies.

The story of the series follows twenty-two year old Jenny Lee, who in 1957 leaves her comfortable home to become a midwife in London’s East End, one of the poorest districts of the metropolis. She is surprised to find that she will be living in a convent, Nonnatus House. Working alongside fellow nurses and the medically-trained nuns, Jenny has her eyes opened to the harsh living conditions of the slums, but she also discovers the warm hearts and the bravery of the mothers. Even after Jenny leaves Nonnatus, she continues to chronicle the lives of the midwives who have become her family.

The casting is perfect and Jessica Raine as Jenny Lee, shines as the inexperienced midwife who gains knowledge, skill, wisdom and maturity through her association with the nuns and fellow midwives, but most of all, the ordinary people of the East End. Helen George as the attractive, vivacious Trixie does well with her role as a “modern and fashionable” young nurse, while Miranda Hart as the tall, ungainly and awkward Chummy is both likeable and believable in her fish out of the water role. Jenny Agutter as the Mother Superior plays in a faultless, understated fashion and her immense acting experience really comes through. The rest of the cast is also exceptional, while the costumes and sets are up to the usual wonderful BBC period drama standards.

The real success of the series is the stories. Each episode deals with a variety of themes, which are replete with humanity at its best and worst. There are moments of high drama and heart-wrenching poignancy, there is humour, grit and sensitivity. The scenes of childbirth are sensitively done, but are also realistic and in some cases quite confronting. However, the whole is tastefully done and quite enthralling. One familiar with the books may balk a little as the screenplay has elaborated the anecdotes of the books. Some of the diversions from the book are to allow the characters other than Jenny Lee to have stronger story lines, which works well for TV. Some of the story lines have been expanded and enhanced for dramatic effect, so they are not relying strictly upon the original author’s memoirs, but rather the imagination of the scriptwriters. Once again this is understandable, given the medium of TV.

This series is entertaining, engaging, utterly watchable and most enjoyable. It proves that if a TV series deals with topics of general interest, has real characters and situations that display ordinary humans trying to make sense of the complicated and messy thing that we call life, it succeeds on multiple levels and makes for satisfying viewing. I recommend this most highly to anyone who wants to not only be entertained, but who also wants to learn a little of the history of mid-20th century London and also of the lives of midwives. We look forward to seeing the rest of the seasons (up to Season 5 in 2016!).

MOVIE MONDAY - THE CONSTANT GARDENER

Eighty years ago if someone contracted pneumonia caused by any one of several types of bacteria, it was quite likely that he or she would die a very rapid and a very unpleasant death. Similarly, at that time, high blood pressure was very poorly controlled and often led to fatal outcomes. Diseases like cancer, which are now controlled by a number of successful drugs, in the recent past quickly killed hundreds of thousands of people a year. Drugs have been developed recently for many common conditions – drugs we take for granted nowadays, and which allow millions of people worldwide to survive potentially lethal diseases. These drugs were only a few decades ago completely unknown. The science of pharmacology (drug discovery, the uses, effects, and modes of action of drugs) has come a long way in the last 50 years or so.

Associated with this immense development of drug therapies is the development of the pharmaceutical industry, a multi-billion dollar industry where multinational companies spend enormous amounts of money to develop, test and market therapeutic drugs. It is a lucrative business and one whose profits have been escalating constantly as more and more new drugs are being discovered and marketed around the world. It is to these multinational drug companies that spend enormous amounts of money to develop and market new drugs that we owe thanks for ridding us of the fatal outcomes associated with many of the killers of the past. We owe them a great deal of gratitude, altruists that they are… Should we really be that grateful? Are they the golden-hearted altruists that they wish us to believe them to be?

Before a new drug is marketed it has to adhere to a set of standards and several safety requirements must be met. Each and every drug has numerous side-effects (some of which are potentially lethal), adverse reactions, risks of causing allergies and other untoward reactions in people who are prescribed the drug. Much research and clinical trials must be carried out in order to test not only the efficacy of a newly developed drug, but also its potential for causing these adverse reactions. Ultimately, the drug must be tested on a human population, as all of the cell culture toxicity trials and animal trials that are carried out will give an indication of how these drugs will work in the human and all favourable preliminary drug trials must be extended to human clinical trials.

The testing of drugs on humans is a thorny area, a veritable minefield of ethical and moral dilemmas, a legally and constitutionally controlled activity, which in most Western countries is regulated to the point of non-viability in many cases. Drug companies that need to develop effective (and profit-making) new drugs spend enormous amounts of money to develop and test these drugs, often taking the clinical trials to developing countries where legislation is laxer and people more willing to “volunteer” take part in the trials (for one another benefit – money, food, or curing of a disease they suffer from). Needless to say that when billions of dollars of profits are involved, some companies are willing to bend a few rules and regulations, some are willing to act illegally and risk thousands of human lives in these clinical trials. People in developing countries are willing to collude with the drug companies, government officials are willing to accept bribes and proper, ethical conduct in drug trials is unlikely to occur under these circumstances. By cutting corners and risking “expendable” lives, profits are increased and the company’s competitiveness in the marketplace is assured. This keeps the shareholders happy and of course, benefits the ultimate end-user of the drug in a Western country who is assured of a cheaper, human-tested drug that can be sold with the pledge of causing few side effects in humans.

For Movie Monday today, I am looking at a film that takes as its subject the dirty and corrupt world of drug development and clinical trials and in its scope attempts to raise public awareness of the magnitude of the problem. It is “The ConstantGardener” (2005), Fernando Meirelles’ film of the book by John Le Carré.  It stars Ralph Fiennes and Rachel Weisz who give good performances in this thriller. The film is also an accusation against big business, corporate corruption and unethical practices in business and government.

I found it to be a disturbing and emotionally challenging film, which contains many images that will stick in any sensitive person’s mind. The depth of the plot of the film is enhanced by several ideas that are also explored: How well do we know our partner in a marriage? How do we express our love in this most special of relationships? Who are our friends and how can we tell? Who can we trust in a difficult situation? How important is our career in our life and where do we place it in our scale of priorities? How active are we prepared to be in the social issues debate and what are we prepared to sacrifice in a quest for justice and fairness on a global level?

Fernando Meirelles has also directed the excellent “City of God” (2002) and he uses a similar documentary-style exposition in this film, and the non-linear plot working works very well, allowing us to delve into the lives of Justin and Tessa slowly, the horrible truth being finally revealed to us as though obscuring curtains are torn down successively. Justin’s journey of discovery and the gaining of wisdom is beautifully recounted and the immensity of his love for his wife is finally affirmed when he follows in her footsteps, realising her own immense love for him. We are challenged by this movie and we are forced to take a stand.

These days, we must realise that nothing remains hidden, no matter which distant part of the world it happens in and we must acknowledge the equal worth of every human life no matter which. We must take responsibility not only for our own actions, but also for the policies and actions of our governments, the ethics of the businesses we patronise and whose products we consume. This film is one that sounds an alarm bell and one that jars our conscience into full alertness and watchfulness…
